• On the day of your appointment, please bring all imaging studies, medical records, and complete list of all medications. We strongly suggest bringing the records in person in ensure a more efficient visit.

How to Prepare for Visit:

• To prepare, please complete the New Patient Form. If you are a voice patient, also complete the Voice-Related Quality of Life Questionnaire and the Glottal Function Form. If you are a swallow patient, complete the Swallow Survey form.
