UNC Health Care Weight Management Services Collaboration
UNC Family Medicine Weight Management Program (WMP) is partnering with the UNC Weight Loss and Lifestyle Medicine (WeLL-Me) Program, the UNC Medical Weight Program (UNC-MWP) at Meadowmont, and the UNC Bariatric Surgery Program to offer more coordinated, interdisciplinary treatment across the continuum of care for patients with obesity and weight-related health problems. Poster Presentation: Is it Possible to Have a Financially Viable, Evidence-Based, Weight Management Program in a Patient Centered Medical Home?
