We Focus on Caring for Adults with the Following Conditions
Given the high demand for infectious diseases services, we must focus on serving adults with complex conditions who benefit most from our team’s expertise, collaborative care, and clinical trials. - HIV
- Medication-based HIV Prevention (Pre-exposure Prophylaxis, or PrEP)
- Patients with severe drug allergies or complex drug-drug interactions impacting treatment of an infection
- Tick or mosquito-borne infection (acquired inside or outside of North Carolina)
- Complex skin and soft tissue infections, including diabetic foot infections
- Infections of bones (osteomyelitis), joints, and joint replacements
- Extrapulmonary infections with non-tuberculous mycobacteria (NTM)
- Recurrent urinary tract infections, especially if drug-resistant bacteria are involved
- Hepatitis C in patients without a history of decompensated cirrhosis
- Recurrent infection with Clostridioides difficile (“C.diff”)
- Patients with latent tuberculosis infection (LTBI) diagnosed prior to starting immunosuppressive medications

Patients who are under consideration for an organ transplant or who have received an organ transplant should be referred to UNC Infectious Diseases Clinic at the Jayson Ray Transplant Clinic.
We Are Unable to Routinely See Adults with the Following Conditions
To ensure access for people with high-priority conditions, UNC Infectious Diseases Clinic does not routinely see adults with the following conditions. These problems are usually well-managed by a primary care provider or by other sub-specialists with expertise in the organ system affected.
- Chronic Fatigue Syndrome
- Concern for chronic EBV (mononucleosis)
- Mold exposure
- Concern for chronic parasitic infection without recent travel or other exposure and/or without diagnostic testing
- Concern for delusional parasitosis or parasites in the skin
- Rash without other diagnostic workup included
- Long COVID (please refer to the UNC COVID Recovery Clinic)
- Chronic wounds or chronic sacral decubitus ulcers (we do not provide wound care and if long-term antimicrobial therapy is indicated we request that patients have care plans in place with surgery, wound care, nutrition, and other colleagues).
- Fevers of duration <14 days or fever without initial workup in place (eg labs or other testing by referring provider)
- For some conditions typically managed by local health departments, eg concern for active tuberculosis, we may recommend accessing local health department services. We typically prioritize consultation for patients with treatment-resistant infection or immunocompromise.
E-Consults
If you are a provider in the UNC Health System, and have a simple question about diagnostic testing or one of the above conditions, you may place an E-consult.
Examples of common eConsult questions:
- Choice of testing (eg seriologic) in setting of suspected or confirmed infection
- Interpretation of serologic testing (eg hepatitis, syphilis) or culture results
- Choice of antimicrobials (eg if allergy or intolerance to first line agent)
- Selection or sequence of vaccination and immunization
