As a specialty practice, to be seen in our clinic all patients must be referred from a physician, physician assistant, or nurse practitioner. This helps ensure that all patients who see us will benefit from the visit. Patients may self-refer for HIV care and HIV prevention (PrEP) but it is helpful to have a referral to ensure that we can access medical records.

How to Place A Referral

Use a faxed referral to refer a patient

If you are a non-UNC physician or mid-level provider and are not using the UNC CareLink portal to refer a patient to the UNC Infectious Diseases Clinic, please fax a completed clinic referral form along with relevant medical records (e.g., clinic notes and test results) to 984-974-4587. Upon receipt, UNC Infectious Diseases Clinic staff and physicians will review the request and identify the appropriate provider. Next, our scheduling team will call the patient to set up the appointment. Should you need to check to see if an appointment has been made, please call 984-974-7198.

Referral Form

Use EPIC to refer a patient

If you are a provider within UNC Health, please place the referral through EPIC. Choose “Ambulatory Referral to Infectious Diseases” and specify “UNC Infectious Diseases Eastowne Chapel Hill”.

Use EPIC CareLink to refer a patient

If you are a non-UNC physician or advanced practice provider you can now use the UNC CareLink portal to directly place referrals, access information about your patients (including scheduled appointments, clinical notes, and more) and send secure messages to UNC Infectious Disease Clinic providers who have seen your patients regarding their care.
