Paperwork for Residency
SOM Consent Form
To request any of the documents listed below you must complete our consent form. We legally cannot send your documents anywhere without a signed consent form; emailed requests are not sufficient for consent, so you will still be required to complete the consent form. Only select the documents you are actively requesting to be sent on the consent form – do not select for us to send documents you don’t need sent. Once your request has been fulfilled, you will be notified by the email address you provide on the consent form. If you request documents that won’t be available until after graduation, you will receive a confirmation that your request has been received then a follow up after graduation that the request was completed.

Transcripts (Official & Unofficial)
Official Transcript: Where you order your official transcript will depend on when you matriculated into SOM. All official transcripts have a base fee of $7.
- If you matriculated after 2014: Your official transcript comes from Parchment – ordering instructions are available here. The fee for your official transcript is $7 plus additional processing fees; please refer to the UNC Main Registrar website for more information.
- If you matriculated in 2014 or before: Your official transcript comes from our office and you can request it via our consent form. There is a $7 fee for all official transcripts which can be paid through the consent form.
Final Transcript: Many GME offices and/or licensure boards will require a Final Transcript – an official transcript including your graduation date which is only available after graduation. If you are ordering your transcript through Parchment you can select the “hold for degree” option which will not send your transcript until it includes your degree conferral date.
Unofficial Transcript: Unofficial transcripts are less commonly needed than official transcripts, but some GME offices need them for students who spent more than 4 years at SOM. These can be requested through our office via the consent form. Unofficial transcripts do not include your degree conferral date, even after graduation.
Education Verification Form
Different states require different forms to verify your MD education. These forms should be submitted to our office via the consent form and are usually sent directly to the medical board/GME office by us – most medical boards will not accept forms submitted by the student/alumnus. Several forms require the student/alumnus to complete a section (usually at the very top) before submission. Some forms can’t be sent to the medical board until after all graduation requirements have been completed or until after graduation – students/alumnus can send them in via consent form ahead of time to be prefilled so that we can submit them as soon as possible once eligible.
Certified Diploma
After graduation our office will have copies of diplomas for all students who graduate in May. We are able to certify these as authentic copies before sending to your GME office/medical board. Very few places require notarized diplomas but we can provide these, though they may have a longer processing time. If you graduate(d) in December or August our office will not have a copy of your diploma, however, if you send us a clear scan or picture of your diploma we can certify this for you.
Letters (Good standing, expected graduation, etc.)
Some GME offices or medical boards will require a letter from our office confirming graduation requirements have been completed, expected graduation, good standing, etc. We will tailor your letter to your needs so be sure to provide any specific requests or language on your consent form or by email to somrecords@med.unc.edu.
Liability Insurance Documentation
You may be asked to provide documentation of liability and/or malpractice coverage while you were a student at SOM. Our office can provide the coverage documents for each year you were enrolled. If you need a claims history pulled, reach out to somrecords@med.unc.edu and we can direct you to where to request that.
Residency Requirements by State:
These requirements are based on the previous year’s requirements and may not be up to date. If you receive conflicting information from your GME office or licensure board please defer to their instructions. Additional requirements from your GME office may be needed. Asterisks denote items less commonly needed.
If your state is not listed I am not currently up-to-date on requirements. Check with your GME office for what you need and we can help you with where to source any documents you aren’t sure about.
North Carolina
- Verification of Medical Education: There are 2 versions of this form, Form A and Form B. Only one form is required but if your paperwork can’t be completed before graduation you may need Form B completed in addition to Form A.
- Form A: Can be completed prior to graduation but after all graduation requirements are fulfilled, including Transition to Residency.
- Form B: Can be completed after graduation.
- Unofficial Transcript*: Students who took more than 4 years to complete their degree may be asked to provide a transcript to explain the extension. Unofficial transcripts can be used for this.
- Official Transcript: Many GME offices will request a Final Transcript – final transcripts are only available after graduation, once your degree conferral is included.
California
- DOCS: As of 1/1/2024 all Medical Board California licensure documents must be uploaded through the DOCS portal and submissions by mail will no longer be accepted.
- Form MED: California’s licensure form is called Form MED. The top section (“Applicant Information”) must be completed by the student/alumni.
- Certified Diploma: California requires a diploma copy to be uploaded. After graduation our office will have copies of all diplomas to certify and upload.
- Official Transcript: If you matriculated after 2014 your official transcript will come from Parchment. From MBC’s website official transcripts from Parchment can be emailed to mbclicensingelectronicnotifications@mbc.ca.gov. If you matriculated in 2014 or before, our office can upload your transcript via DOCS.
Colorado
- Letter of expected graduation*: Not all Colorado residencies require this, but several have requested a letter confirming good standing and expected graduation date. Be sure to list any specifics you’re told the letter needs to include on your consent form or by email!
Florida
- Certified Diploma: Florida requests a diploma copy to verify your graduation. They will not accept letters of expected/anticipated graduation; letters are only viable after graduation if your diploma is not available yet.
Louisiana
- Certificate of Dean/Registrar: This is Louisiana’s education verification form – do not send it to any SOM Deans as our office completes it. The top half needs to be completed by the student/alumnus, include a passport quality photo, and be notarized on the attached photo. We have a few notaries on staff who can assist with this – reach out to somrecords@med.unc.edu for more information. The form can be completed after all graduation requirements have been fulfilled, including Transition to Residency.
Massachusetts
- Medical Education Verification: There are 2 versions of this form depending on if you’re submitting it before or after graduation; many GME offices want you to complete both versions.
- Form A: This form can be submitted prior to graduation but after all requirements are completed. Students/alumni must complete the first section (“Applicant Instructions”) before submitting via a consent form.
- Form B: This form can’t be submitted until after graduation, but can be sent in via a consent form anytime for pre-filling. It does not require any sections to be completed by the student/alumnus.
Michigan
- LARA Certification of Medical Education: This is Michigan’s medical education verification form. Students/alumnus should complete the top section (“Applicant Information”) before sending to our office via a consent form. The form requires an upcoming graduation certification, so it can’t be completed until all graduation requirements are done, including Transition to Residency.
New York
- Certification of Professional and Preprofessional Education: This is Michigan’s medical education verification form. Students/alumnus should complete the entire first page, being sure to sign, before sending to our office via a consent form. This form can’t be completed or submitted until after graduation, but you can submit it ahead of time for pre-filling.
Pennsylvania
- Verification of Medical Education: Students/alumni must complete the Section 1/Applicant Information before submitting via the consent form. This form can’t be submitted until after graduation but can be sent in to our office ahead of time for pre-filling.
South Carolina
- Certification of Medical or Osteopathic Education: This is South Carolina’s education verification form. It cannot be completed until after graduation and usually needs to be physically mailed in. The top half (“Applicant Information”) section must be completed and signed by the student/alumnus. The form also requests the student’s/alumnus’ hometown be listed, so please try to provide that when you submit the consent form. If not, we’ll reach out to verify it.
Virginia
- Certificate of Professional Education (Form B): This is Virginia’s education verification form. It cannot be completed until after graduation, but you can submit it ahead of time for pre-filling.
Washington
- Letter of Intent to Graduate: Washington requires a certification of expected graduation letter sent directly to the board, usually by email. This can only be provided once you have completed all graduation requirements (including Transition to Residency)
