Sub-Acute Services

Rex and UNC offer state-of-the-art patient care in Wake County as members of the UNC Health Care family, a not-for-profit integrated health care system based in Chapel Hill.

As part of this partnership between UNC and Rex, our physiatrists provide evaluation, monitoring, and follow-up Monday through Friday at two sub acute facilities in Wake County:

Program Mission

Our short term, sub acute programs in Wake County are designed for individuals who are discharged from the hospital but are not quite ready to return home. We offer 24-hour on-call coverage with physiatrists and physician extenders for medical issues and rehabilitation. With the ability to tailor therapy using a well managed approach, we serve patients who may benefit from a flexible rehabilitation plan after hospitalization.
