Credentialing, Licensing, & Verifications

The UNC SOM Registrar’s Office is responsible for releasing information to licensure boards, hospitals, clinics, etc. that need to verify that you received your MD degree from the UNC School of Medicine.

Please complete the SOM Consent Form (Electronic Form or PDF Version for Mailing) and return it to the Office of Student Affairs by any of the following means:
