Clinic Appointment: In most cases, your referring physician will have sent your medical records to your doctor at UNC. However, if you have other records you should bring them with you including your medications and x-rays if applicable.
New Appointment: If you have never been seen by a UNC Liver Program physician before, your doctor must call 919-966-2516 and request a UNC Liver Program registration form or obtain the form from our website. In order to provide the most efficient and thorough care your medical records and a copy of your insurance card have to be included with the registration form. The referring doctor and patient will be notified of the date and time of the appointment.
Return Appointment: If you have been seen by the UNC Liver Program before, you may request a return appointment by calling 919-966-2511. Please have your UNC medical record number and the name of the physician previously seen available.
Where you will be seen: The UNC Liver Program has clinical facilities at UNC Hospitals. When you get your appointment slip in the mail, the location, date and time of your appointment will be noted. If you cannot make the appointment, we request that you call and reschedule at least 48 hours in advance so that other patients may be offered your appointment time.
Parking Information: If your appointment is in the main hospital/GI Medicine Clinic, you should park in the Visitor’s Parking Deck across the street from the hospital complex. Valet parking is also available at the front entrance of the hospital.
