Refer a Patient
As a specialty practice, all patients to be seen in our clinic must be referred from a physician, physician’s assistant, or nurse practitioner. This helps ensure that all patients who see us will benefit from the visit.
Before Placing a Referral
Referring providers, thank you for your referral and your trust in us! Before placing a referral, please review our “who we care for” page which outlines the conditions we are able to best care for.
Use EPIC CareLink to refer patients and receive results
If you are a non-UNC physician or mid-level provider you can now use the UNC CareLink portal to directly place referrals, access information about your patients (including scheduled appointments, clinical notes, and more) and send secure messages to UNC gastroenterologists regarding your patients’ care.

Refer a patient to the gastroenterology clinic
Please fax a completed clinic referral form (PDF Version Available Here), along with relevant medical records (e.g., clinic notes and test results) to 984-974-3414. Upon receipt, a gastroenterologist will review the request and identify the appropriate provider.
Next, our scheduling team will call the patient to set up the appointment. Finally, we will fax you the patient’s appointment time and mail the patient information about his/her appointment.

Refer a patient to the liver clinic
Please fax a completed clinic referral form (PDF Version Available Here), along with relevant medical records (e.g., clinic notes and test results) to 984-974-3414. Upon receipt, a hepatologist will review the request and identify the appropriate provider.
Next, our scheduling team will call the patient to set up the appointment. Finally, we will fax you the patient’s appointment time and mail the patient information about his/her appointment.

Refer a patient for a Fibroscan
Please fax a completed Fibroscan referral form (PDF Version Available Here) along with relevant medical records to 984-974-3414. Upon receipt, the records will be reviewed, and our scheduling team will contact the patient to set up an appointment.

Refer a patient for a GI procedure or motility test
Please fax a completed GI procedure referral form (PDF Version Available Here) or GI motility referral form (PDF Version Available Here) to 919-966-8764.
Our scheduling team will call and ask the patient a short series of questions before scheduling the appointment. We will then fax you the patient’s appointment time and mail the patient information about his/her appointment.

Refer a patient for biofeedback
Pelvic Floor Biofeedback is provided by UNC pelvic floor physical therapists at both the Hillsborough Medical Office Building and the UNC Center for Rehabilitation Care.
To refer a patient for pelvic health PT including pelvic floor biofeedback, please complete and fax a Pelvic Health PT/Biofeedback Referral form to 984-974-9789. Appropriate referrals are patients with fecal incontinence, constipation due to pelvic floor dyssenergia, and chronic proctalgia.
UNC Physical Therapy schedulers will contact the patient and schedule them at the most appropriate location depending on patient insurance, location, and patient preference.
