SOM New Hire & Termination Checklists, Forms, & Policies:
- Clinical Department Hiring Exception Request Form - for SPA and EPA positions
- Policy and Procedure for SOM New Hire and Terminiation Checklists - for SPA and EPA employees
- SOM New Hire Checklist for SPA Employees, Temps and Students - to be completed within the first thirty days of employment
- Exit/Termination Checklist for all SOM Employees (EPA, SPA, Temporary and Student Employees) - to be completed by the employee's last workday. The HR Facilitator should begin working to complete form within thirty days of the employee's planned departure date or as soon as a departure date is known.
