Nominations can be submitted by any faculty, staff, and students from UNC SOM and surrounding AHEC sites. Where appropriate, the Awards Committee recommends that awards come from peer nominations via letters from the Course Director, Faculty Course Members, and/or Department Chairs or Division Chiefs. Each nomination requires a nominating or supporting letter from the Department Chair or Division Chief. The nomination letter should provide information on the teaching contributions and evaluations of the nominee, if available and applicable to the specific award. Other support letters could be solicited by the nominator from other course faculty and/or students. The nomination cycle will run from January 16, 2025, through April 1, 2025.

A completed nomination packet includes the following:

  • A Nomination Letter from the Nominator (unless self-nominating*)
  • A SOM formatted CV from the Nominee in PDF format
  • A Letter from the Division Chief, Department Chair, or Center Director of the Nominee
  • OPTIONAL Additional Letters of Support (one additional letter required if self-nominating*)

 

*Self-Nominations: Interested educators should not wait to be nominated if they feel they qualify for an award.   All potential awardees are encouraged to request their own letters and submit their packet as a self-nomination.  Self-nominations should at a minimum include the Division Chief/Chair/Center Director letter AND one additional letter of support from someone who can speak to nominee’s skills and accomplishments as an educator.

Teaching Award Nomination Form

  • Enter your first and last name(s).
  • Enter your email address. We will send a confirmation message to this email address after you submit your nomination.
  • List the name of the person you are nominating. Self-nominations are encouraged.
  • Enter the email address of the person you are nominating. When you click submit, they will receive a message notifying them of their nomination. They will not be notified of your name nor which award for which you nominated them.
  • Select the award for which you are nominating the individual. If you wish to nominate them for multiple awards, you must complete separate submissions for each award.
  • Accepted file types: pdf, Max. file size: 98 MB.
    Upload your nomination letter in PDF format. If self-nominating, this should be a letter of support from someone who can speak to your skills and accomplishments as an educator.
  • Accepted file types: pdf, Max. file size: 98 MB.
    Upload the nomination letter from the department chair/division chief/center director/program director letter in PDF format. If you are the department chair/division chief/center director/program director, simply upload your letter again.
  • Accepted file types: pdf, Max. file size: 98 MB.
    Upload the CV of the person you are nominating here. If you have trouble acquiring this, upload a blank document and email aoe@med.unc.edu for assistance.

      Submit any other letters of support/documentation that you believe would be valuable to the committee in reviewing your nomination.
