Nominations can be submitted by any faculty, staff, and students from UNC SOM and surrounding AHEC sites. Where appropriate, the Awards Committee recommends that awards come from peer nominations via letters from the Course Director, Faculty Course Members and/or Department Chairs or Division Chiefs. Each nomination requires a nominating or supporting letter from the Department Chair or Division Chief. The nomination letter should provide information on the teaching contributions and student evaluations, if available, of the nominee. Other support letters could be solicited by the nominator from other course faculty and/or students. The nomination cycle will commence on this webpage, beginning January 26, 2022 through April 6, 2022.

A completed nomination packet includes the following:

  • A Nomination Letter from the Nominator
  • A SOM formatted CV from the Nominee in PDF format
  • A Letter from the Division Chief or Department Chair of the Nominee
  • OPTIONAL Additional Letters of Support

Teaching Award Nomination Form

  • Enter your first and last name(s).
  • Enter your email address. We will send a confirmation message to this email address after you submit your nomination.
  • List the name of the person you are nominating.
  • Enter the email address of the person you are nominating. When you click submit, they will receive a message notifying them of their nomination. They will not be notified of your name nor which award for which you nominated them.
  • Select the award for which you are nominating the individual. If you wish to nominate them for multiple awards, you must complete separate submissions for each award.
  • Accepted file types: pdf, Max. file size: 100 MB.
    Upload your nomination letter in PDF format.

    Upload the nomination letter from the department chair/division chief letter in PDF format. If you are the department chair/division chief, simply upload your letter again.
  • Accepted file types: pdf, Max. file size: 100 MB.
    Upload the CV of the person you are nominating here. If you have trouble acquiring this, upload a blank document and email for assistance.

      Submit any other letters of support/documentation that you believe would be valuable to the committee in reviewing your nomination.
