CPD Policies and Forms

Here you will find the CPD Policies regarding the administration of CME under the providership of the School of Medicine of the University of North Carolina at Chapel Hill, as well as some of the most common forms.  For questions about these policies and forms, please contact the CME Coordinator, Doug Hudson, via email or at 919-445-4302.

To Request Credit

  • Application. UNC CPD has strict requirements regarding the types of activities for which we can provide credit.  If you are interested in credit, please contact the CME Coordinator, who will determine if UNC CPD can meet your needs.  The CME Coordinator will send you the application at that time if appropriate.
  • Intent to Continue Credit.  This form is only for regularly scheduled series currently receiving credit.

  • CPD Course Checklist.  For use with one time and annual events and activities with multiple identical sessions.
  • CPD Course Checklist Guide.
  • CPD RSS Checklist.  For use with regularly scheduled series such as grand rounds, M&M conferences, case presentations, and journal clubs.
  • CPD RSS Checklist Guide.
  • Enduring Material Guide. Enduring materials are complex and the paperwork needs to be tailored to the individual activity, so we do not currently have a guide.  Please contact the CME Coordinator to discuss the details of your activity.
  • CPD Planning Worksheet.  This should only be completed at the request of the CME Coordinator.
