Schedule An Appointment

To make a new patient appointment, please call 984-974-5703. Your insurance may require a referral and pre-authorization. Please bring your medical insurance card, pharmacy benefits card, and any medications you are taking to your appointment, along with any prior imaging (x-rays, CT scans, etc.) you may have had done.  Most new patients will also be scheduled for Pulmonary Function Testing on the same day or prior to the visit with the physician. If testing is scheduled prior, it may be at one of several testing locations based on availability and convenience for the patient. 

Our providers are able to see you in person at Eastowne or one of our other clinics in the area, or when medically appropriate, via video visit or telephone visit. You may inquire about virtual options at the time that you schedule your appointment, though due to federal regulations, virtual visits may only be conducted with patients who are located in North Carolina.

Please inform us as soon as possible if you are unable to come to your scheduled appointment, so we may reschedule you and make the appointment slot available for another patient.  If bad weather inhibits you from attending your visit, we may be able to convert your face-to-face visit to a virtual visit with your provider.
