As a specialty practice, all patients to be seen in our clinic must be referred from a physician, physician’s assistant, or nurse practitioner. This helps ensure that all patients who see us will benefit from the visit.

Before placing a referral

Please review our Pulmonary Subspecialty Care page which outlines the conditions we are able to best care for.

Information to include with referral

To make the visit most informative, we ask that you please include:

  • Specific referral diagnoses or symptoms as this determines what PFTs will be ordered
  • Reports of chest imaging from the last 5 years (along with images via PowerShare or on CD – link to radiology for patients page)
  • Results of all available pulmonary function tests
  • Any other relevant laboratory tests

How To Place A Referral

Use EPIC to refer a patient

If you are a provider within UNC Health, please place the referral through EPIC. Choose “Ambulatory Referral to Pulmonology” and select “UNC Pulmonology at Eastowne”.

Use EPIC CareLink to refer a patient

If you are a non-UNC physician or mid-level provider, you can use the UNC CareLink portal to directly place referrals, and access information about your patients (including scheduled appointments, clinical notes, and more).

Use a faxed referral to refer a patient

If you are a non-UNC physician or mid-level provider and are not using the UNC CareLink portal, please fax a completed referral form or send with the patient all pertinent records, including reports of chest imaging (along with images via powershare or on CD), pulmonary function tests, and any other relevant laboratory tests.

E-Consults

For physicians within the UNC System, if you have a focused question that specialists can answer using solely the information available in Epic without seeing the patient, consider placing an e-consult. Before placing the e-consult, please review this site for more information and guidance on the process.
