Improving health outcomes and quality of life.

Recent studies have shown that people with severe mental illnesses die earlier than the general population, mainly because of poorly recognized and treated physical health conditions and illnesses. It is also increasingly recognized that co-occurring physical and behavioral (mental) illnesses contribute disproportionately to the high cost of medical care.

There is growing interest in developing models of integrated care for people with mental illness, which will improve health outcomes and quality of life, while reducing overall costs.

Physical Health Integration

The Center is actively developing models of integrated care, which treat both psychiatric and medical illnesses. With a grant from The Duke Endowment, we have established a Health Home for persons with severe mental illness, one that emphasizes health monitoring and prevention, health and wellness programs, and coordination of behavioral and physical care.

The Center’s Health Home provides primary care by reverse-co-location in the UNC STEP Carrboro clinic, transitional care, health and wellness programs, and outcome monitoring.

Behavioral Health Integration

Many psychiatric illnesses, including depression, anxiety disorders, and substance abuse are diagnosed and treated in primary care settings. The Center is at the forefront of efforts to partner psychiatrists with primary care physicians to improve the recognition and treatment of these disorders.

Carolina Advanced Health

The Center collaborates with Carolina Advanced Health to provide behavioral health consultation as part of a new model of primary care that offers a comprehensive team-based approach for patients with chronic medical conditions. Carolina Advanced Health was created by Blue Cross and Blue Shield of North Carolina and UNC Health Care to deliver a more personalized level of care.
