We are currently accepting applications for the 2018-2019 academic year. Please submit all application material prior to the October 1st Deadline.

Applicants will be expected to participate in ERAS (Electronic Residency Application Service). This program participates in the NRMP Primary Care Sports Medicine Fellowship match process. Only applications containing the following will be considered complete.

1. Completed ERAS
2. Curriculum Vitae
3. 2 originally signed letters of recommendation
4. Letter of Evaluation from your Medical School's Dean
5. Official Copy of your Medical School Transcript
6. Letter stating good standing and expected completion of residency from the program director

***A third letter of recommendation can be substituted for the Letter of good standing from the residency program director if the candidate has completed specialty training prior to applying to the fellowship.

The deadline for receipt of application is October 1, 2017. It is the applicant’s responsibility to verify receipt of all materials. Invitation only interviews will be conducted October to December.
