Welcome to the DVT/PE Transition Clinic! Our goal is to provide transitions of care for patients with newly diagnosed deep vein thrombosis or pulmonary embolism. Your health care provider may refer you to this clinic if you were recently diagnosed with a blood clot.

The clinic is located within the Benign Hematology clinic at UNC Eastowne.
100 Eastowne Drive
Chapel Hill, NC

At your visit, your provider will:

  • Confirm your diagnosis and that appropriate treatment has been started.
  • Make sure that your blood work is up-to-date and any medications you have been prescribed for your blood clot are safe for you.
  • Check other medications you are on and ensure drug interactions are checked.
  • Help with more complex situations such as patients who also have cancer, blood disorders, history of certain surgeries or conditions, and more.
  • Ensure you have access to your medications including financial assistance when needed.
  • Provide education on risks and benefits of medications.
  • Provide education on hormone therapy and counseling on contraception when relevant.
  • Discuss what to do for surgeries, procedures, and other situations if you are on an anticoagulant.
  • Consider additional screening tests that may be recommended.

You will most likely see a physician assistant or nurse practitioner at your appointment, who will work with a supervising physician. A medical assistant and pharmacist will also help with your visit and any questions that come up regarding the appointment or medications.
