  1. To reassess the need for long-term anticoagulation;
  2. To receive information about relevant studies that have been published in the last year and of ongoing studies that may influence treatment or be of relevance for other family members;
  3. To learn of ongoing studies that you may be eligible for and may want to enroll in;
  4. To learn about new anticoagulant drugs that may have become available;
  5. To discuss thrombophilia issues that may be relevant (family connections, parents, siblings) – the use of contraceptives, hormone replacement therapy;
  6. To discover medical issues that you may not recognize;
  7. To have an early intervention if you have developed post-thrombotic syndrome (assessment of need for compression stockings, vascular radiology intervention, home compression pump, etc);
  8. To bring young female patients up-to date on state of the art anticoagulant management during pregnancy (identifying the right anticoagulant drug, right dose) and choice of birth control options;
  9. To assess if you have a history of PE,  chronic lung disease (pulmonary hypertension);
  10. To become aware of regional and national thrombosis patient advocacy group activities;
  11. To become aware of resources for information regarding blood clots and thrombophilia.
