Aim

Increase the number of spirometry completed in the Rex Primary Care of Cary clinic by 10% and increase symptom assessment in patients with spirometry-confirmed COPD to 50% by August 31st, 2018.

Project Accomplishments

The COPD project has been spread system-wide to over 61 primary care practices. The composite measure is called COPD uncontrolled treatment and includes:

  1. Rescue inhaler prescribed
  2. Symptom assessment regularly (depends on last symptom score)
  3. Spirometry
  4. Appropriate treatment based on symptom score
  5. Inhaler education annually
