Aim

Increase the number of patients with cystic fibrosis transitioned from pediatric to adult care at the appropriate time.

Project Accomplishments

  • Met with the Lung Transplant Team to develop ideas for spreading the Transition program to transplant patients
  • Recognized as an approved ABP MOC Project providing Part IV credit for physicians
  • Dr. Goralski was invited to join the Transition Advisory Committee for CF R.I.S.E. to work on spread to other hospitals
  • Dr. Goralski became part of a national working group on transition through the American Thoracic Society
  • Efforts spurred the CF care team into forming a Purple Belt Team and completing a Kaizen event on clinic flow

  • Poster presentation at the 2017 North American Cystic Fibrosis Conference
  • Included as a “featured booth” at the 2017 and 2018 North American Cystic Fibrosis Conference Quality Improvement Fair
