NICU: Decrease the rate of unplanned extubation events from 0.54 to less than 0.5 per 100 ventilator days by September 30, 2026

PICU: Maintain below the solutions for Patient Safety centerline of 0.34 unplanned extubations per 100 ventilator days by September 30, 2026
