Quality Improvement Program (QIP) for Family Medicine Residents

Participation Requirements

Complete the following:

☐ Participant must be involved in the creation, development and oversight of the project implementation throughout the QI effort, while working with Residents

☐ Participate in direct care of the patients involved in the project.

☐ Participant must meet with others involved in the QI team to discuss interventions.

☐ Participant must review practice performance data and use the data to develop tests of change and determine ways to sustain the successful aspects of the project.

Reflection

☐ Provide feedback and reflection about the project via a survey at the end of the project, including the following questions:

  • What change did you personally make in your practice?
  • How did this change impact patient care in your practice?
  • What did you learn as part of participating in this QI effort?
  • Explain how you plan to sustain the changes you made to your practice because of this QI effort.
