UNC Medical Center ED-to-Hospice Transitions

Participation Requirements

Complete the following:

☐ Participate in update meetings for at least 6 months of this project in order to offer feedback and hear about small tests of change that may occur.

☐ Participants must review reports reflecting on rates of hospice referrals for patients presenting to the ED.

☐ Participate in activities (meetings, tool design, PDSA cycles, etc.) designed to increase knowledge and utilization of the ED-to-hospice pathway available.

☐ Participants must will be available throughout the study duration to respond to questions and concerns from ED team members working with this patient population.

Reflection

☐ Provide feedback and reflection about the project via a survey at the end of the project, including the following questions:

  • What change did you personally make in your practice?
  • How did this change impact patient care in your practice?
  • What did you learn as part of participating in this QI effort?
  • Explain how you plan to sustain the changes you made to your practice because of this QI effort.
