Quality Improvement Poster

Participation Requirements

To receive credit, the poster must meet the following requirements for Quality Improvement:

☐ Have an adequate and appropriate Aim Statement/SMART Goal

☐ Have an adequate and appropriate measures that are aligned to determine if change occurred and relates directly to the Aim Statement/SMART Goal

☐ Have adequate and appropriate interventions that have been aligned to impact the measures and were implemented into practice

 

Reflection

☐ Provide feedback and reflection about the project via a survey at the end of the project, including the following questions:

  • What change did you personally make in your practice?
  • How did this change impact patient care in your practice?
  • What did you learn as part of participating in this QI effort?
  • Explain how you plan to sustain the changes you made to your practice because of this QI effort.
