Participation Requirements

a. Complete the following
a. Participating physicians will provide well child visits to children from birth to age 6 for 6 months.
b. Participating physicians will review data related to kindergarten readiness.
c. Participating physicians will participate in identifying actions and interventions to promote kindergarten readiness of their patients.
d. Participating physicians will reflect on outcomes of kindergarten readiness promotion efforts and consider whether efforts have improved outcomes.
