Referring Physicians

Instructions for referral to the Medical Weight Program using EPIC

  1. In the orders and medications panel search for “Obesity.”
  2. Click on the “database” tab.
  3. Select AMBULATORY REFERRAL TO OBESITY MED. PROGRAM.
  4. Alternatively you could search for “REF198.”

If you are a non-UNC provider, please call 984-974-6519 or fax the referral to 984-974-2924 for UNC Medical Weight Program or place orders (referrals) via the UNC CareLink account access (available to all non-UNC providers/practices).
