We provide comprehensive care for adult patients with various forms of kidney diseases. Our team is also involved in clinical trials, clinical and basic science research, and training the next generation of nephrologists.
To ensure access to patients with complex kidney conditions, UNC Nephrologists may not be able to routinely see patients with certain kidney conditions which can be managed by primary care providers. Please review the kidney conditions that are appropriate for either nephrology evaluation in clinic, an e-consult to nephrology, or managed by primary care providers/other subspecialties. Given the high demand for nephrology services, we must focus on care of individuals with complex form of kidney diseases who would benefit the most from our team’s expertise, multidisciplinary care, and clinical trials. Acute Kidney Injury-

- Patients are seen in Multidisciplinary clinic by providers from multiple specialties including nephrology, urology and nutritional services
- Patients with significant and persistent following electrolyte abnormalities
- Patients with kidney disease who are currently pregnant or considering pregnancy are seen in a multidisciplinary clinic
- If a patient is planning to or currently receiving care from UNC Maternal Fetal Medicine (MFM) and would like to be seen in the MFM Nephrology clinic located at the Vilcom Location in Chapel Hill please call 984-215-5000 or place via Epic referrals (Ambulatory Referral to Maternal-Fetal Medicine) and put “MFM-nephrology clinic” in the referral line.

Kidney Transplant Evaluation
Please have your physician (either primary care of specialist) fax a completed clinic referral form (Kidney Transplant Review and Referral Form), along with relevant medical records (e.g., clinic notes and test results) to 984-974-5200. Upon receipt, a clinical staff member will review the request and identify the appropriate provider. Next, our scheduling team will call you to set up the appointment. Finally, we will fax your physician a copy of the appointment time and mail you information about your appointment. Should you need to check to see if an appointment has been made, please call 984-974-0888.
Kidney Transplant Referral Form
Post Kidney Transplant Clinic
Please have your physician (either primary care of specialist) fax the relevant medical records (e.g., clinic notes and test results) to 984-974-0888 along with a face sheet mentioning “Referral for Post Kidney Transplant patient”. Upon receipt, a clinical staff will review the request and identify the appropriate provider. Next, our scheduling team will call you to set up the appointment. Finally, we will fax your physician a copy of the appointment time and mail you information about your appointment. Should you need to check to see if an appointment has been made, please call 984-974-1764.

Referring providers may consider another subspecialty or refer to our linked care pathways. If you are a provider in the UNC Health System and have a simple question, you may place an e-consult if the criteria is met.

- Urinary tract Infection
- Pyelonephritis
- Hydronephrosis (Consider Urology referral)
- Simple Cyst(s) (without hereditary kidney disease)
- General Kidney Health
- Angiomyolipoma (Consider Urology referral)
- Adrenal adenoma (Consider Endocrinology referral)
- Presumed or diagnosed Pheochromocytoma (Consider Endocrinology referral)
- Preventive Kidney Health

- Penile or bladder abnormalities in absence of kidney abnormalities
If you are a provider in the UNC Health System and have a simple question, you may place an E-consult for the following conditions. Providers outside of the UNC health system can review the suggested guidelines/Carepathway links.
- eGFR 45–60 ml/min/1.73m2 and/or UACR 30-299 mg/g (with comorbid DM). Review CKD carepathways and CKD management goals.
- eGFR 30–60 ml/min/1.73m2 and/or UACR 30-299 mg/g (without comorbid DM). Review CKD carepathways and CKD management goals.
- Minor electrolyte abnormalities. Review hyperkalemia management.
- Use of SGLT2i in CKD. Review guidelines.
- Risk of use of Iodine or Gadolinium-based contrast in CKD
- Medications use in CKD. Review CKD management goals.
- Perioperative medications management with CKD
- Hyperuricemia/Gout and CKD
- Use of Cystatin C vs. Creatinine vs. measured GFR estimation
- Hypertension – uncontrolled on < 3 medications. Review the hypertension handbook.
- CKD stage II in pregnancy (eGFR > 60ml/min prior to pregnancy or Cr < 1.0mg/dl before 20 weeks)
- Proteinuria less than 0.5g by 24hr urine or urine protein-to-creatinine ratio in pregnancy
- Others
