Important information for your appointment

Please have the following information ready:

  • Name and address of the referring physician
  • Correct spelling of patient’s full and legal name
  • Patient’s date of birth
  • Social security number and insurance information including the name of the insurance carrier, name of the policyholder (subscriber name), and the policy number
What to bring with you to your appointment
  • UNC patient identification number if it has been issued.
  • A list of current prescriptions, over-the-counter medications, and supplements that you take. (You may bring your bottles with you.)
  • Your insurance card and provider referral forms if required by insurance.
  • Information about your medical and surgical history.
  • Any relevant films or records you may have related to your current problem. When possible, please send these to us PRIOR to your appointment to ensure that we have all materials necessary for your visit.
  • A list of questions that you have about your condition and/or symptoms.
No Show, Late and Cancellation Policy
At UNC Health, we value your time, and we work hard to maintain the flow of appointments in our clinics. Our No-Show policy helps ensure all our patients can see their providers as close to their appointment times as possible.

To reschedule or cancel your appointment, contact your clinic at least 24 hours in advance by calling their office or using My UNC Chart.

Policy Details

Our no-show and cancellation policy applies to missed appointments, which include:

  • No-show: You don’t arrive for your appointment, and you don’t reach out to cancel.
  • Late arrival: You arrive 15 or more minutes past your appointment time.
  • Late cancellation: You cancel your appointment less than 24 hours before your appointment time.

Insurance

UNC Health contracts with most health insurance companies, including the ones listed here. While we do our best to keep this list of in-network insurance up-to-date, it often changes. Please contact the Neurology clinic, your Neurology doctor, or your insurance company to determine coverage.

Clinicians

The UNC Department of Neurology provides consultative and patient care services for patients referred by primary care physicians, other specialists, and other neurologists. Following our evaluation, a detailed report is sent back to the referring physician.

Adult Referrals | Pediatric Referrals

NOTE: Due to limited staffing and other considerations, we are currently unable to provide services for the following conditions: low back pain, primary psychiatric disorders, traumatic brain injury, and fibromyalgia.

Expediting Scheduling

If your patient has undergone any of the below studies/evaluations, please provide us with a copy of those reports should you have a copy. Otherwise, please encourage your patients to acquire and bring them to their visit with us.

  • MRIs
  • CT scans
  • Laboratory and genetic testing
  • EMG
  • EEG
  • Sleep studies
  • Other neurodiagnostic testing
  • Visit notes
  • Procedure notes (e.g. lumbar puncture, and injections)
UNC CareLink

UNC Health web-based referral portal provides online access for referring providers for the following features:

  • A detailed overview of your patient’s medical record
  • Secure messaging with UNC Health providers regarding your patient’s care
  • Online referrals for outpatient and ancillary services
  • Access to scheduled appointments, medications, allergies, and outpatient & inpatient notes (including admissions, discharges and emergency services)
  • Helps with CMS Meaningful Use and patient-centered medical home efforts
