How to Refer a Patient:

Providers who have access to UNC CareLink can submit a referral online.

If you would like to enroll, click on the link and select “Request an Account”. You may wish to download an overview of the referral portal or view a step-by-step guide on how to sign up.

If you do not have a UNC CareLink account, please fax a completed referral form to the appropriate fax number (see below).

In order to avoid a delay of the referral process please be sure to answer all parts of the referral form including:

  • the specific question (reason for consult);
  • complete, accurate contact information for the patient;
  • and insurance information if applicable.

New Patient Referral Process

Referrals are first reviewed to determine the level of urgency and UNC’s ability to fulfill the consult request. We have streamlined our processing of urgent referrals to be completed within 48 hours of receipt should the referring provider/office contact our office to inform us of the reason for urgency. We ask that the referring provider be available to discuss the reason for urgent consideration with one of our reviewing physicians, as this will ensure proper triaging and timely scheduling when appropriate.

Non-urgent referrals are otherwise processed within three to five business days often by a clinician to ensure the patient receives the most appropriate care within our clinic. Our staff will contact the patient directly to schedule an appointment.

Services We Are Not Equipped To Treat

Adult Neurology – We are currently unable to offer consultative services for lower back pain, spinal injury, TBI, or Fibromyalgia.

Child Neurology – We are currently unable to offer consultative services for concussion, learning difficulties, behavior problems, ADHD, or autism (except if there is a concern about comorbid epilepsy).

Spine Center

The Center prefers referrals to come electronically via UNC Carelink. Start the process by entering an Ambulatory Referral to the Spine Center. There is an option after this to specify a specialty if desired; the Center has spine specialists from PM&R, Ortho and Neurosurgery. If you do not have UNC CareLink, you may submit a referral by faxing (919) 966-9094 or by calling (984) 974-4200. Please note that referrals are not necessarily required.

Conditions for Which We are Unable to Provide Services

Due to limited staffing and other considerations, we are currently unable to provide service for conditions listed below. The following provides the contact information of specialists who may be able to offer expertise for services that we are not equipped to treat at this time.

Expediting Referral Review
  • In order to expedite the care of your patient, please provide the specific reason for the consult request and fully complete the referral form. Also, please include the patient’s current contact and insurance information.
  • Providers who have access to UNC CareLink may submit a referral request online. If you would like to enroll, click on UNC CareLink and select “Request an Account.” You may wish to download an overview of the referral portal or view a step-by-step guide on how to sign up.
  • If you don’t have a UNC CareLink account, please fax a completed Referral Form, including the specific referral question and pertinent records to (984) 974-2285. If you have any questions, please call (984) 974-4401 or email neurologyreferrals@unchealth.unc.edu.

If your patient has undergone any of the below studies/evaluations, please provide us with a copy of those reports should you have a copy. Otherwise, please encourage your patients to acquire and bring them to their visit with us.

  • MRIs
  • CT scans
  • Laboratory and genetic testing
  • EMG
  • EEG
  • Sleep studies
  • Other neurodiagnostic testing
  • Visit notes
  • Procedure notes (e.g. lumbar puncture, and injections)

EMG Studies at UNC Hillsborough

UNC Neurology at Hillsborough has the capacity to perform select EMG studies, with short appointment scheduling turnaround times available. Referrals can be placed via Epic using the “EMG” or “Electrodiagnostic” orders. Please select “Hillsborough” as the location performing the study.

If outside of UNC, referrals may also be faxed to 984-974-2285.

Studies We Cannot Complete

Due to technical limitations at this stage, we are unable to complete the following studies at Hillsborough:

  • Single fiber EMG
  • Repetitive nerve stimulation test
  • Abdominal muscles EMG, thoracic paraspinal muscles EM, diaphragmatic EMG, pelvic floor muscles EMG
  • Patients who require a mechanical lift to transfer to exam table
  • EMG & ultrasound study
