Upload Compliance Documents – Qualtrics UPLOAD Link

The 4 documents listed below MUST be on file with the APP Center prior to the start of a student’s clinical rotation in order for the student to be cleared to begin the clinical rotation.

The 4 documents must be provided to the APP Center via the following Qualtrics link:  Qualtrics LINK for compliance doc Uploads 2022

  1. Student’s Professional Liability COI (if covered under a policy that covers all students, submit that COI.  If individual policy, submit that COI)
  2. LMS Transcript (Student must “Export to PDF” or “Print to PDF”  their LMS transcript from their LMS account after completing the HCS Regulatory Compliance Module and EPIC Training Modules.
  3. Compliance Summary (ONE of the following)
    1. Summary Immunization Report from school’s compliance database or vendor that includes the same information listed on the table below (OR)
    2. Letter of Good Standing on school letterhead that includes the same information listed on the table below (OR)
    3. Completed Passport Form
  4. COVID Vaccine Card
