Our Mission

SHAC Social Needs Navigation is a free service offered to the community to help guide patients through the medical system and reduce barriers to care. We aim to provide resources, navigation, and support across the domains of communication, transportation, housing and utilities, financial assistance, food security, and emotional and physical wellbeing.

Location & Hours

We operate alongside the medical clinics on Wednesdays from 6:00PM-10:00PM at the SHAC Clinic. It is located at:

About Us

Social Needs Navigation is a clinic integrated into many of SHAC’s medical services and specialty clinics and serves as a resource for nonclinical aspects of health. Our navigators provide distress screenings and offer a wide variety of resources for all socio-economic and supportive needs. Navigators receive training on local and online resources and commonly assist with needs such as:

  • Financial Assistance
  • Housing
  • Transportation
  • Employment
  • Physical Safety
  • Food Security
  • Emotional and Mental Wellbeing
  • And More
