About Us

Who Are We?

SHAC Outreach strives to bring healthcare services directly to you in your own community. Our mission is to provide patient-centered care for those who might otherwise lack access to healthcare services and to ensure that our patients obtain follow-up care that meets their every need. We seek to prevent acute medical issues through a focus on preventative care and lifestyle modification counseling.

We offer monthly walk-in clinics in two locations: St. Thomas More Church (STM) in Chapel Hill and Urban Ministries of Durham (UMD).

What Services Do We Provide?

Our monthly walk-in clinics at St. Thomas More Church (STM) and Urban Ministries Durham (UMD) always offer blood sugar and blood pressure screenings, medication reconciliation, lifestyle modification counseling, and medication questions. We also offer a preventative health consultation service, in which participants receive personalized plans with recommendations from the US Preventive Services Task Force and CDC Immunization Schedule in order to stay healthy for the future. Additional services are offered at some walk-in clinics, and may include physical therapy as well as other evaluations. At both STM and UMD, we have established partnerships with many local clinics including SHAC Clinic, Samaritan Health, and Lincoln Health for patients needing follow-up care from our screenings. We also have a list of resources for patients seeking any form of medical care, and we will work to put you in touch with a local clinic or program that meets your needs.

We always offer Spanish interpretation services, especially at our Saint Thomas (STM) location.

Our Locations

Urban Ministries of Durham (UMD):

• Clinics at UMD are held in the Multi-Purpose Room after dinner is served on weekday nights.
• Clinics start around 6:45 pm and last until 8:15 pm.

• Clinics at STM are held in the St. Anthony room after the Spanish Mass on Sundays.
• Clinics start around 2:45 pm and last until 4:15 pm.
