Our Mission

The “Bridge to Care” Clinic provides continuing medical care to patients with chronic conditions such as diabetes, hypertension, hyper/hypothyroidism, etc. Medical visits are provided during regular clinic hours on Wednesday evenings. Our ultimate goal is to bridge patients to more long-term healthcare. We are also able to connect patients to other resources within SHAC such as SHAC Wellness, the Medication Assistance Program (MAP), SHAC Dental, etc.

Location & Hours

We operate alongside medical clinic every Wednesday from 6:00PM to 10:00PM at the SHAC Medical Clinic.

About Us

As a part of Bridge To Care, you will meet with medical professionals who specialize in chronic care. We can also refer patients to a local provider and, if necessary, apply for charity care. SHAC has relationships with other community resources and can help refer you to free/reduced priced clinics that will also help you with your conditions.

Make an Appointment

As of now, appointments are required. To schedule an appointment or if you have a question, please contact Bridge to Care through one of the following:

  1. Please call 919-904-4302, wait for the line to finish ringing and then leave a message stating your full name and phone number, and we will get back to you. To reduce the risk to patients and caregivers, we will over telehealth appointments by phone or video chat for patients with concerns that can be evaluated remotely.

What is the best way for patients to contact SHAC Bridge To Care (BTC)?

You can contact us at 919-904-4302 and leave a voicemail with your name, date of birth, and reason why you’re calling. We will get back to you within 48 hours. If it is an emergency, please hang up and call 911.


I am a Bridge To Care (BTC) patient and I need a medication refill, what should I do?

Call us at 919-904-4302 and leave your name, date of birth, medication(s) you need refilled, and preferred pharmacy name and location.


What if I do not know if I am a Bridge To Care (BTC) patient?

Call us at 919-904-4302 and leave your name, date of birth, and reason for calling. We will connect you with the correct program.
