About Us
To make an appointment, please fill out the survey below under “Make an Appointment”. All new patients will first receive a free radiology appointment for a panoramic radiograph and a free screening appointment to determine their dental needs. Eligible patients may then receive up to three free treatment appointments.
Services provided at SHAC Dental Clinic include:
• Dental radiographs (X-rays)
• Dental cleanings
• Fillings
• Extractions (excluding wisdom teeth)
• Anterior root canals (front teeth only)
Services not provided at the SHAC Dental Clinic include:
• Wisdom tooth extractions
• Molar root canals
• Retreatment of previous root canals
• Prosthodontic treatment (crowns, bridges, partial dentures, dentures, and implants)
• TMJ-related pain and disorders
• Orthodontic treatment (braces and Invisalign)
• Pediatric dental care

• Previous SHAC patients who have already used all three treatment appointments
• Patients dismissed from the UNC Adams School of Dentistry
• Patients who have received care in the UNC Adams School of Dentistry student clinics within the past three years (specialty clinics excluded)
Make an Appointment
We are an appointment only free dental clinic.
Appointment Request form: SHAC Dental Appointments
By submitting this form you acknowledge that you understand SHAC’s appointment policy and clinical guidelines.
Due to high demand for our free dental services, our new patient form will open in the months of December and July, and new patients will be contacted in chronological order based on when their form is submitted.
To be added to our new patient list, please complete the above form. The form is open annually from July 1 through July 31 and December 1 through December 31. Please submit only one entry per person during each enrollment period. Duplicate entries will be deleted.
Your submission will remain active until the next enrollment period. If you submit your form in July, your entry will remain on our list until December 1. If you submit your form in December, your entry will remain on our list until July 1. If selected, one of our clinic coordinators will contact you to schedule an appointment. We will make up to three attempts to reach you. If we are unable to contact you after three attempts, your name will be removed from the list. If you are not contacted before the next enrollment period (June or December), you must submit a new submission when the form is open to remain eligible.
SHAC students comply with HIPAA regulations and patient medical records are maintained according to accepted confidentiality standards. If you have questions or need assistance completing this form, please email SHAC_DentalClinic@dentistry.unc.edu or text 984-999-1654.
Attention prospective patients:
Please bring a list of your current medications.
If you are diabetic, please know your most recent A1C.
If you are asthmatic please bring your inhaler.
If you are taking an anticoagulant and need extractions please bring a note from your primary care physicians that your INR is stable.
Please note: if you have elevated blood pressure or blood glucose level we will not be able to treat you in our clinic.
Frequently Asked Questions
Will I receive treatment the first time I visit?
No, our clinic flow process includes at least 3 appointments, with treatment only occurring after a new patient screening.

If you are experiencing a dental emergency, please call UNC Dental School Urgent Care Department at (919) 537-3737 between 8AM and 5PM.
Can I receive more than one dental treatment in a clinic night?
Since there are more patients than we have time to treat, we can only provide one treatment per patient at each clinic night.
Can you extract my wisdom teeth?
This depends entirely on each individual. We have to take x-rays and do a clinical exam to determine if we can extract a wisdom tooth.
